The ambulance was instantly called, but their team examined and declared him dead on the spot.
This unfortunate incident was followed by a HSE investigation. The HSE noted that the company was familiar with the possible danger of blast due to blockages in the mixer and the subsequent accumulation of steam pressure inside it. However, the company kept quiet on the matter and did not take any measures to correct this fault.
The Court was also informed during the proceedings that a similar kind of accident had occurred in May 2006. At that time, a male worker got seriously hurt while using the same machine. The blast that had happened had twisted a metal-covered external wall, forcing it out by 50cm. Had the machinery fault been rectified, this fatal incident could have been totally prevented, informed the HSE.
